Aftercare after an exosome treatment: a guide for patients

What the patient does in the hours and days after a session directly affects the result. This guide brings together the recommended aftercare following an exosome or biostimulator treatment, ready to share with the patient.

Why aftercare matters

An exosome or biostimulator treatment sets in motion repair and remodeling processes that continue for days and weeks. During that time the skin is more receptive, but also more vulnerable: to sun, heat, friction and irritating products. Good aftercare:

  • Reduces discomfort and how long redness lasts.
  • Lowers the risk of complications such as post-inflammatory hyperpigmentation.
  • Protects the result of the treatment.

This guide is designed for practitioners to share with patients. Specific instructions from the practitioner always take precedence over these general recommendations.

The first 24 hours

After an exosome treatment —for example Exosome + GHK-Cu or Ginseng + Centella Exosome—:

  • Avoid intense sun exposure. If you must go out, wear a hat and broad-spectrum sunscreen.
  • Avoid saunas, steam baths, hot tubs and intense heat.
  • Do not use irritating cosmetics: scrubs, retinoids, acids (glycolic, salicylic, lactic), concentrated vitamin C or products with alcohol or fragrance.
  • Do not wear makeup, unless the practitioner says otherwise.
  • Do not touch or rub the treated area.
  • Avoid intense exercise, which raises temperature and blood flow in the skin.
  • Sleep with a clean face and, if possible, on your back the first night.

The first 48 hours after a biostimulator

After Exosome + PLLA or another biostimulator, in addition to the above:

  • Avoid excessive heat, saunas and intense sun exposure for 48 hours.
  • Avoid vigorous massage of the treated area for 48 hours.
  • Be aware that erythema, mild swelling or, temporarily, small self-limiting inflammatory nodules may appear. Mention them at your follow-up visit.

The home routine during treatment

Step What to use What to avoid
Cleansing A gentle cleanser without harsh sulfates or fragrance Brushes, scrubs, very hot water
Moisturizing A simple cream without alcohol or perfume Products with many actives at once
Protection Broad-spectrum sunscreen every morning, reapplied Going out unprotected
Actives Only those recommended by the practitioner Retinoids, acids and irritating depigmenting agents in the days afterward

When to resume your usual routine depends on the type of treatment and how your skin responds; the practitioner will tell you when.

Sun protection: the non-negotiable rule

Ultraviolet radiation keeps inflammation going, breaks down collagen and encourages dark spots, especially in skin that is repairing itself. Recommendations:

  • Broad-spectrum sunscreen every day, even when cloudy or indoors with lots of natural light.
  • Reapply every 2 to 3 hours if exposed.
  • Add a hat and sunglasses outdoors.
  • In higher phototypes, sun protection is even more important to prevent post-inflammatory hyperpigmentation.

Makeup

  • Wait at least 24 hours, or as long as the practitioner advises.
  • After microneedling, the microchannels need to close before applying products that could contaminate the skin.
  • Use clean brushes and sponges.
  • Choose simple, mineral products for the first few days.

Exercise, heat and water

  • Intense exercise: avoid for the first 24 hours, and for up to 48 hours after a biostimulator.
  • Chlorinated pools and the sea: wait until the skin has recovered to avoid irritation and infection.
  • Showers: lukewarm water, without aiming the jet directly at the face.

What is normal and what is not

Normal:

  • Mild redness and a feeling of heat in the first hours.
  • Slight tightness or dryness.
  • Mild swelling, especially in thin-skinned areas.
  • Fine flaking in the days after intense microneedling.
  • After biostimulators, small temporary nodules that resolve on their own.

Contact the clinic if you notice:

  • Pain that increases instead of decreasing.
  • Intense local heat, discharge or pus.
  • Extensive crusting or blisters.
  • Fever or feeling generally unwell.
  • Redness that gets worse after 48 hours.
  • Nodules that are painful, growing or not improving.
  • Any unexpected or persistent reaction.

In the event of an unexpected reaction, stop any application and seek professional assessment.

Aftercare by treated area

  • Face: avoid sleeping on your side against the pillow the first night, skip heavy sunglasses that press on the treated area, and postpone facial waxing or hair removal for a few days.
  • Neck and décolleté: avoid necklaces, scarves or rough fabrics that rub the skin, and remember to apply sunscreen here too, since this area gets a lot of sun.
  • Hands: wear gloves for cleaning tasks with chemical products and reapply sunscreen after every wash, because it comes off easily.
  • Arms, abdomen, buttocks and legs: wear loose clothing for the first 24 to 48 hours, avoid friction from very tight garments, and do not apply slimming creams, body scrubs or self-tanners to the area until the practitioner says so.

Common questions at home

  • "My cream stings a little when I apply it." Stop that product and use only a simple, fragrance-free moisturizer; if the stinging persists, let the clinic know.
  • "I can feel small bumps in my skin." After a biostimulator, small temporary nodules may appear; do not massage them on your own and mention them at your follow-up. If they hurt, grow or turn red, contact the clinic.
  • "I forgot my sunscreen one day." Resume it straight away and be extra careful over the following days; consistency is what prevents dark spots.
  • "Can I take a painkiller?" Ask the practitioner which medication is appropriate for you before taking anything.
  • "Can I bring the next session forward?" It is not advisable: the intervals allow the skin to finish repairing and collagen to form.
  • "My skin looks better already. Can I skip the remaining sessions?" Early improvements come mostly from hydration and calmer skin; the deeper changes in texture and firmness need the full cycle, so it is worth completing it.

Between sessions

Exosome and biostimulator treatments work in cycles. Between sessions:

  • Keep your routine simple and use sunscreen every day.
  • Stick to the schedule: 14 days between repair sessions with Exosome + GHK-Cu, or 4 weeks between Exosome + PLLA sessions, unless told otherwise.
  • Let the clinic know if you started a new medication, had an infection or had another procedure.
  • Do not combine treatments at other clinics on your own without telling your practitioner.

Habits that support the result

  • Good sleep: skin repair intensifies during rest.
  • Hydration and nutrition with enough protein.
  • No smoking: tobacco reduces tissue oxygenation and collagen production.
  • Stable weight, especially in body treatments.
  • Maintenance as agreed: new collagen holds up better with periodic sessions.

Quick checklist for the patient

  1. No intense sun, sauna or steam for 24 hours (48 hours after a biostimulator).
  2. No makeup or irritating actives for 24 hours.
  3. Gentle cleansing, a simple moisturizer and daily sunscreen.
  4. Do not touch, rub or massage the area (48 hours after a biostimulator).
  5. No intense exercise for 24 to 48 hours.
  6. Call the clinic if pain increases, there is discharge, fever or an unexpected reaction.
  7. Stick to the session schedule and maintenance plan.

For practitioners: how to hand over these instructions

Giving aftercare in writing —printed or sent by message— reduces doubts and phone calls, and leaves a record in the clinical file. Going over it aloud with the patient at the end of the session improves compliance. A follow-up call or message at 24 to 48 hours helps catch any reaction early and builds trust.

A closing message for the session

Before the patient leaves, it helps to sum up the most important point in one sentence: "Today and tomorrow, no sun, heat or strong products; your skin is working." A short, memorable instruction gets followed far more often than a long list nobody reads again.

Summary

Aftercare is part of the treatment. In the first 24 hours —48 after a biostimulator— the priority is to avoid sun, heat, friction and irritating actives; afterwards, keep a simple routine and daily sun protection, stick to the session schedule and get in touch at any warning sign.
